Company Story
Heart Failure Solutions was founded to close the gap between what heart failure programs are supposed to do — catch decompensation early, follow up consistently after discharge, and capture the revenue that supports the work — and what most programs actually run day to day. We built a documented, trainable system from direct clinical experience, so a program's effectiveness doesn't depend on any single physician remembering to do the right thing every time.
Methodology Note
HFS's Integrated Heart Failure Management System (IHFMS) is described in a pending U.S. patent application co-invented by our founders, covering inpatient identification, hemodynamic monitoring, transitional care coordination, remote physiologic management, and reimbursement optimization as one integrated system. Patent-pending status confers no issued rights yet — we mention it because it reflects the rigor behind the methodology, not as a marketing claim beyond what it means.
