Hemodynamic Monitoring
We train your team to run a clean selection, implementation, and monitoring workflow across the leading hemodynamic-monitoring platforms — CardioMEMS™ HF System (Abbott), Cordella™ PA Sensor System (Edwards Lifesciences), and ZOLL® HFMS (non-invasive). HFS is device-agnostic: we don't sell devices or receive manufacturer compensation for device volume. What we provide is the workflow around the device — standardized selection criteria, a defined review cadence, and a clear escalation pathway for when a physician doesn't respond to an out-of-range reading.
Transitional Care Coordination
Rescue Diuretic Management
A structured 7-/14-day post-discharge follow-up workflow, built around Medicare's Transitional Care Management (TCM) billing codes. We train providers and staff on TCM documentation requirements and the RVU/financial difference between a standard office visit and a TCM code — revenue most practices are already entitled to but frequently under-capture.
We help practices enable rescue-diuretic prescribing (FUROSCIX®, ENBUMYST™) as a same-day option, backed by an objective, hemodynamics-guided titration protocol — driven by pressure-trend data rather than symptom report alone, and run day-to-day by a trained designated staff member under physician-approved standing orders.
