Heart Failure Solutions
About

Built by cardiologists, not consultants.

We founded Heart Failure Solutions to close the gap between what heart failure programs are meant to do and what they often do day to day.

Company story

A system, so it does not depend on memory.

A good heart failure program catches decompensation early, follows up consistently after discharge, and captures the revenue that funds the work. In practice, many programs manage only some of that, some of the time.

We built a documented, trainable system from direct clinical experience, so a program's effectiveness does not rely on one physician remembering every step.

Methodology · Patent pending

The Integrated Heart Failure Management System.

One integrated approach covering five areas, described in a pending U.S. patent application co-invented by our founders.

  1. 1
    Inpatient identification
  2. 2
    Hemodynamic monitoring
  3. 3
    Transitional care coordination
  4. 4
    Remote physiologic management
  5. 5
    Reimbursement optimization

The application is pending, and pending status grants no issued patent rights. We mention it to show the rigor behind the methodology.
